Getting Started

Step 1

Schedule a No-Cost Initial Consultation

To schedule your initial consultation please call 720 - 470 - 0010 (Boulder) or 970-744-9292 (Fort Collins). We ask for a credit card number to hold your appointment.  If your appointment is completed as scheduled , there is no charge. However, if you cancel your appointment and do not inform us 24 hrs ahead of time, we will need to bill you for the time you reserved.

Step 2

Fill out the Intake Questionnaire

The following Intake Questionnaire is available for you to download and print. It is helpful for you to have the questionnaire filled out prior to your initial consultation.  Please answer what feels comfortable to you. Step 3

Initial Consultation

Your initial consultation will be scheduled with La Luna Center's Clinical Director, Dr. Malia Sperry. At this meeting you can discuss your history and current needs and explore treatment options that might be appropriate for you. This meeting can also provide you with a feel for La Luna Center's philosophy and answer any questions you may have about eating disorders and the recovery process.

During your consultation we can collect your insurance information and check your benefits. If you would prefer to submit your insurance information online, you can do so by clicking here.

Step 4

Begin Treatment

After your initial consultation you will understand your treatment options and be able to make decisions about the type of treatment that is right for you. You may begin that treatment process as soon as you are ready.

Intensive Outpatient Program Specific Form

This form is available for download if needed after your initial consultation has been completed. For individuals enrolling in our Intensive Outpatient Program we ask that medical clearance be obtained prior to beginning treatment.  This clearance helps us assure your medically stability and appropriateness for outpatient treatment.  The Adobe Acrobat reader is needed to open the document, if you need to download the Adobe reader then please click here.
