Fill out the Intake Questionnaire
The following Intake Questionnaire is available for you to fill out online. It is necessary for you to have the questionnaire filled out prior to your initial consultation. Please answer what feels comfortable to you.
STEP 3
Initial Phone Consultation
Your initial phone consultation will be scheduled with La Luna Center's Clinicial Supervisor. In this meeting you can discuss your history and current needs and explore appropriate treatment options. This meeting will also provide you with a feel for La Luna Center and answer any questions you may have about eating disorders and the recovery process. Insurance benefits and costs of treatment will also be provided to you.
STEP 4
Begin Treatment
After your initial consultation, you will understand your treatment options and be able to make decisions about the type of treatment that is right for you. You may begin that treatment process as soon as you are ready.
